When the Heart Sends an SOS
Every heartbeat relies on a precise molecular mechanism involving Troponin — a regulatory protein complex embedded in cardiac muscle fibres. Under normal conditions, troponin stays inside heart cells. But when cardiac muscle is starved of oxygen and cells begin to die — as in a Myocardial Infarction (heart attack) — the cell membranes rupture and Troponin T and Troponin I spill into the bloodstream in measurable quantities.
This makes troponin exquisitely specific: it is only elevated when actual heart muscle damage has occurred. Modern High-Sensitivity Troponin (hs-cTn) assays can detect injury at the picogram level, enabling accurate heart attack diagnosis or rule-out within just 1–2 hours of chest pain onset — a revolution compared to older markers like CK-MB that took 6–12 hours.
